About

Laboratory Services at Forks Community Hospital are available on an inpatient and outpatient basis.

Our outpatient phlebotomy hours are Monday-Friday, 7:00AM to 5:00PM. Weekends and Holidays 7:00AM to 2:00PM.

While no appointment is needed, our staff will work to accommodate specific patient’s testing needs outside of routine phlebotomy hours. Be sure to call the lab and let our staff know how we can help.

For outpatient phlebotomy services, patients must first register before having a lab test performed. Upon arriving at the hospital, sign in at the front reception and admitting desk and from there, a friendly staff member will direct you to the laboratory.

We are a full-service laboratory which supports inpatients, outpatient, surgical, and emergency services. Our experienced and professional staff connects our medical providers to a greater network of specialized testing and pathology services.
The laboratory is equipped with the latest instrumentation to support a wide range of laboratory tests and staffed by Nationally Certified Medical Technologists and Nationally Certified Medical Laboratory Technicians.

FAQs

Do I need to fast before having my blood drawn?
Continue taking medications unless specifically directed not to by your medical provider. If you have questions, please call your provider.

Be sure to drink plenty of water.

Fasting is a requirement primarily for Lipid (cholesterol) profiles and glucose testing. Fasting means no food or drink (except water) for 8-10 hours before your blood draw.
How do I get the results from my tests?
Forks Community Hospital uses MyChart electronic health records. Patients may access their test results by signing-in to their private and secure patient portal. If you have not signed up for MyChart, please let the staff at the admissions desk know and they will be happy to assist.

Questions about your results? We encourage you to follow up with the ordering provider. This allows them to make interpretations which are specific to the reason for performing the tests and provide further instructions, such as changes to medicines, further follow-up care, or additional testing.

You can also access your results by completing a Medical Records Request. Please allow 24 hours for routine tests and one-week for special testing, pathology, and cultures.
Can I use the Forks Community Hospital Laboratory if my orders are from an out-of-area provider?
Yes! You can have your outpatient laboratory tests done at Forks Community Hospital’s laboratory even if your provider is located out of the area. Please bring a copy of your lab order or have your provider fax the order to 360-374-3164.

Because you can receive high-quality diagnostic imaging services in Forks, there is no need to travel a long distance to get a clear picture of your health!

FCH provides a wide range of services from general X-ray, Fluoroscopy, Computed Tomography or CT to Ultrasound Echocardiography, 3D Mammography, Bone Density (DEXA), Mobile MRI, and Mobile Nuclear Medicine.
