Forks Community Hospital Physical Rehabilitation Services offer hospital-based inpatient and outpatient Rehabilitation Services for all ages. At FCH, we do not take a “one-size-fits-all” approach. Each patient is thoroughly evaluated to determine the most appropriate course of care.

West End Outreach Services
West End Outreach Services (WEOS) provides a wide range of mental health and substance use disorder services. All providers are registered, certified, or licensed by the Washington State Department of Health. Private insurance and Medicaid are accepted. Other arrangements can be made depending on need.
