How We Care for You
We understand that patients managing one or more chronic conditions require a special level of care coordination. Whether it is assistance with managing medication, providing referrals to specialists, making recommendations for adhering to a special diet, connecting patients with therapy and other services, we are here to help patients improve their quality of life and provide peace of mind.

Your long-term health and well-being are essential, as is establishing care with a primary care professional. Forks Community Hospital has three Rural Health Clinics, and our team of providers, including physicians, physician assistants, and advanced practice nurses, provide a wide range of preventative and chronic disease management services.
