Welcome to Forks Long Term Care, a small, homelike facility serving the residents of western Clallam and Jefferson counties.
Striving to furnish compassionate care to your loved ones, we provide a comfortable, home-like atmosphere while offering close medical supervision. With only 20 residents, we offer a personal connection to our residents by honoring their preferences and encouraging them to their highest practical level of functioning.
Spacious, semi-private rooms, some with patios, offer residents the opportunity to surround themselves with some of their own furnishings. Natural skylights in the hallways brighten the environment while a cheery fireplace in the parlor creates a warm, gathering atmosphere.
Birthdays are honored as well as holidays and significant community events.
Family pets are welcome to visit with proper documentation, but please call ahead.
A skilled, professional nursing staff provides individualized care to all residents. Together with our residents, their families, and our staff, we plan and deliver high quality care and services.
Should the need arise, the facilities of the Hospital are immediately available. A medical social worker and mental health professionals are available to both residents and their families to ease the adjustment to this new life style.
